I am a total gore hound, I have seen everything from troma films, to older hammer flicks, to the august underground series, to the guinea pig series. I have come to the culmination that the three most completely FUCKED up movies of all time are (in no particular order) Salo (120 days of sodom) - an Italian flick based off of the 18th century French novel by le Marquis de Sade. www.imdb.com/title/tt0073650/Philosophy of a Knife - A 4 hour opus dedicated to the horrific happenings of the Japanese unit 731. As I type this I am only halfway through this film and am already confident to put it on this list. Note that this is a totally disconnected art/brainwash film (almost similar to the style of Shinya Tsukamoto's Tetsuo - The Iron Man) and if you aren't familiar with film noir then this most likely won't make much sense, but will still make you want to cry. www.imdb.com/title/tt0961119/Men Behind The Sun - A Japanese made movie about the same subject as the one previously mentioned, unit 731, shorter in length it makes up for story line, continuity, and color. www.imdb.com/title/tt0093170/Anyone care to add to this list?

i wrote a huge like 10 or 12 page paper on Unit 731. i was obsessed with reading and learning about it for awhile. its so fucked up that i dont know if i want to watch a movie about it though

I feel like Clockwork Orange is worthy of this list. Plus there's a ton of good B-movies that fit the bill. Try Tokyo Gore Police.

i'm totally being mainstream here, but the only movie i've ever had to force myself to finish has been the cell, because of the intestine cranking scene... eugh. seems like i need to broaden my horizons though haha.

Pink Flamingos (For the Dog Shit eating Scene) DONT watch salo if shit eating is gross to you haha I will now list the 3 most fucked up movies with little to no gore as these films tend to focus more on acting, plot development and story line than exploitive gore scenes. Mysterious Skin - The horrors of molestation, Gregg Araki style, really really hard to watch for most men. Excellent film. www.imdb.com/title/tt0370986/Dancer in the Dark - A musical, out of this almost 3 hour long movie there are really only 2 scenes of violence and no profanity if I recall, this movie is a musical written by and starring bjork, I know you're asking yourself "How could this be one of the most fucked up movies?" Yeah? Fucking watch it. This movie seriously made me cry. www.imdb.com/title/tt0168629/Noriko's Dinner Table - A totally emotional bizarre film about girls who run away from home, to join what you come to learn is part of a "suicide club," (which is the title to its prequel) and become "emotional" prostitutes. I'm taking a subversive film class. pretty much everything we watch is disturbing. Nothing has been really gory, but we havent had "death night" yet. Thats when we're watching three films, one of which is of an autopsy room in Pennsylvania, another is inside a slaughter house, and I dont remember what the third one is. I think its about concentration camps. I'll report back. If you're religious, The Devils is pretty disturbing. There's a scene called "the rape of Christ" which was thought to have been destroyed, but someone found the film and released a bootleg version a few years ago. Its disturbing in a lot of ways, but there is one scene where he is raping an unconscious girl, and its paralleled with her getting a tracheotomy, and its disgusting. Cronenberg's Crash is sexually disturbing. Theres a scene where the two main male characters lick/make out with each other's fresh tattoos. And another scene where a woman's car accident scar is used like a vagina. And lots of other gross shit.

Martyrs is great, and which august underground did you watch? Mordum is the one i suggest. If film quality is REALLY that big of an issue watch Penance, It's shot with an hd cam. Also, if youre into torture/murder gore porns like the AU series, check out the "Slaughtered Vomit Dolls" and "Slaughtered Vomit Dolls : RE GOREGITATED" (This has one of the most insane scenes I have ever seen in it)

I would like to add Jenifer and Pelts by Dario Argento. Both were his episodes of Masters of Horror and both are just plain fucked up. Jenifer more-so with the fact that *SPOILER ALERT* they not only kill off a child of young age but show her feeding on the naked corpse of it *END SPOILER.* Dario Argento has a tendency to make surreal and often disturbing films.
Also it is far from a horror flick, but Secretary deals with some pretty fucked up subject matter. I watched it on IFC the other night and I highly recommend it. Yet another well done film regarding BDSM and sado-masochism.
Mainly your best bet for finding movies like these are sticking with the Asian (mainly Japan and South Korea) and European (Mainly Italy and France) markets.

Also I forgot to throw in Combat Shock. One of the rare and few serious films released through Troma.
I also forgot to add 13: Game of Death. This is one of the releases from Dimension extreme that I consider to be one of the hits (its either hit or miss with the company). Its basically about an average citizen who gets a strange phone call telling him that if he performs 13 tasks that he will win a substantial amount of money. Each task gets harder and more fucked up than the last, the movie doesnt wait either and it starts getting pretty disturbing once the third challenge hits. If he doesnt complete it then he loses everything. Again, I can not stress how fucked up the challenges get in this movie. One of the few movies to ever actually gross me out a bit.

He reviews some of the guinea pig flicks, and nekromatik 2 was another one he reviewed. The synopsis sounded so fucking mind blowing to me. I was disturbed more by reading that I think. I dig lots of 70's exploitation/post apocalyptic type films, but I've never really watched a lot of fucked up art/exploit/gore/porn/whatever stuff. I get disturbed by it WAY to easily.

anything that marquis de sade touched is disturbing
but, are you strictly interested in gore? el topo is not necessarily gory, but definitely disturbing.
Irreversible has about a 9 minute rape scene.

I only watched about half of Irreversible, my copy didnt have any captions =[ I got to the brutal beating at the gay club scene and got bored with it.
Have you read any Marquis de Sade? If you haven't then you really have no idea. I'm about 400 pages deep in to the 120 days of sodom, its very difficult to sit and read for more than a few pages at a time.

Teeth is actually a really good film, it makes good uses of lighting and suggestive moods, the plot and dialog are silly, the gore is minimal, but effective, however teeth is not disturbing or horrifying in any right.
